How do I get a copy of a birth certificate?

How to do it

  1. Determine eligibility: Ohio birth records are restricted. Eligible: the person named (if adult), parents, legal guardian, legal representative. Others must show legal interest.
  2. Choose your method:
  3. IN PERSON (Local Health Department/City Health Registrar - FASTEST): Ohio has 100+ local vital statistics offices; find your nearest at odh.ohio.gov/vital-statistics/how-to-order; bring valid photo ID; same-day or next-day service in most cases; fee typically $21.50
  4. IN PERSON (ODH Walk-in - Columbus): Ohio Department of Health, Bureau of Vital Statistics, 35 E. Chestnut St., Columbus, OH 43215; walk-in hours Mon-Fri 10 AM-2 PM; fee $21.50; NOT same-day - requests submitted online or in person processed within 5 business days
  5. BY MAIL (ODH): Complete application; include check or money order payable to "Ohio Department of Health"; mail to: ODH Vital Statistics, P.O. Box 15098, Columbus, OH 43215-0098; MAIL ONLY: checks and money orders (no cash or credit cards for mail); 4-6 week processing
  6. ONLINE (ODH online portal): Apply at ohid.ohio.gov (Ohio's integrated online portal for state services); online requests processed within 5 business days
  7. PHONE/VitalChek: VitalChek authorized vendor; additional fees
  8. Submit with valid photo ID copy and proof of qualifying relationship

County-specific variations apply

Ohio's 100+ local vital statistics offices (county health departments, combined health districts, city health departments) each hold records for events in their jurisdiction and provide faster in-person service than the state office. Note: some local offices hold records from their jurisdiction's formation date; older or pre-registration records may only be at the state level. Death records older than 50 years and birth records older than 125 years are transferred from ODH to the Ohio History Connection for archival.

  • $21.50 search fee is NONREFUNDABLE as of January 1, 2025 (ORC 3705.24) - applies whether or not record is found.
  • State ODH mail: 4-6 weeks; use local health department or ODH online/walk-in for faster service.
  • ODH walk-in does NOT provide same-day service - still takes 5 business days.
  • Death records older than 50 years and birth records older than 125 years → Ohio History Connection (ohiohistory.org).
  • Marriage and divorce records NOT held by ODH state office - go to county Probate Court (marriage) or Clerk of Courts (divorce).
