How to do it

  1. Note: Connecticut death certificates are NOT restricted - anyone age 18 or older may purchase a certified copy; no proof of relationship required
  2. Choose your method:
  3. IN PERSON: Visit the Town Clerk/Vital Records office in the town where the death occurred (most towns process same day during business hours); or the State Vital Records Office walk-in hours
  4. BY MAIL: Complete the death certificate request form; include payment; mail to the town where the death occurred or to the State Vital Records Office
  5. ONLINE (expedited): Order through VitalChek (only approved third-party vendor for CT DPH)
  6. Provide name of decedent, date of death, and town where death occurred
  7. Pay the fee
  8. Receive certificate

  • Connecticut death certificates are OPEN RECORDS - anyone 18+ can purchase a certified copy without proving relationship; this is more permissive than most states.
