How do I get a copy of a death certificate?

How to do it

  1. Determine eligibility: Nevada death records are CONFIDENTIAL. Eligible requesters: immediate family (spouse, parents, children, siblings, grandparents), legal guardian/representative, funeral directors, insurance companies and financial institutions with demonstrated interest, and others with documented tangible interest.
  2. Determine which office holds the record:
  3. - Clark County deaths 1988+: Southern Nevada Health District (Las Vegas)
  4. - Clark County deaths before 1988: State Office in Carson City
  5. - Washoe County deaths (all years): Northern Nevada Public Health (Reno) - includes records from late 1800s to present
  6. - All other Nevada counties (any year): State Office in Carson City
  7. Choose your method:
  8. IN PERSON (Washoe County - Reno): NNPH, 1001 E. Ninth Street, Reno, Building B, 1st floor; window hours: 8 AM-12:45 PM and 2-4 PM Mon-Fri; bring valid photo ID
  9. MAIL / DROP-OFF (Clark County): Southern Nevada Health District Vital Records; 2-4 weeks processing; NO same-day service
  10. MAIL (State DPBH): 4150 Technology Way, Suite 104, Carson City, NV 89706
  11. ONLINE (VitalChek): authorized vendor; additional fees
  12. Submit with valid photo ID and proof of qualifying interest

Same structure as birth certificates - Clark County and Washoe County have their own health district offices. All other Nevada counties use the state office. No same-day service at Clark County or state office.
