How to do it
Determine eligibility: Utah birth records restricted. Eligible: the registrant (if adult), parents, legal guardian, legal representative, spouse, children, grandchildren, siblings, grandparents, or others with documented legal interest (per Utah Code 26B-8-125).

IN PERSON - County/Local Health Department (FASTEST): Many Utah counties have local vital records offices that can issue birth certificates same day for births in their jurisdiction; Davis County, Salt Lake County, Utah County, Weber County, and other counties have local vital records offices; bring valid photo ID; fees slightly vary by county
IN PERSON - Utah State Office of Vital Records (Salt Lake City): 288 North 1460 West, Salt Lake City, UT 84116; business hours Mon-Fri; bring valid photo ID; walk-in service available
ONLINE (VitalChek or secure.utah.gov/vitalrecords): State offers online ordering; $22 state fee + online convenience fee ($4) + optional expedited processing ($15) + optional expedited shipping ($30); card accepted
BY MAIL (State Office): Complete application from vitalrecords.utah.gov; include front AND back of valid photo ID (enlarged, legible copy); check or money order payable to "Vital Records"; mail to: Office of Vital Records and Statistics, Utah DHHS, P.O. Box 141012, Salt Lake City, UT 84114-1012

County-specific variations apply
Many Utah county health departments issue birth and death certificates for events in their county. Davis County (Clearfield) $25 birth/$35 death. Salt Lake County, Utah County, Weber County, Cache County, Washington County, Box Elder County, and others have local vital records offices. Very small rural Utah counties may not have local vital records issuance - use state office.
